Information submitted through the inquiry form is used to respond to your inquiry, prepare for an introductory discussion and communicate about the services or partnership opportunity you asked about.
Website hosting and service providers
CTFA uses website hosting, email and survey application providers to operate this website, store assessment responses and respond to inquiries.

Assessments and your information
The three pillar assessments open in CTFA’s hosted survey application. Your name, email address and organization are required. Role and phone number are optional.
CTFA saves your contact information when you begin and your answers as you select “Save & continue.” Authorized CTFA staff can review both partial and completed questionnaires to prepare for a conversation and follow up on your inquiry.
A session cookie allows you to continue on the same device for up to seven days. Providing a phone number permits a call about your inquiry; it does not subscribe you to marketing texts or a newsletter.
Please use general descriptions and leave out employee names, claim numbers, medical records and other confidential claim information. To request a correction or deletion, email info@claimthefutureacademy.org.
